Insurance Information

We accept most insurance plans including Medicare. Please provide your insurance information to our staff when you schedule your first appointment so we can verify that your services will be covered.

Appointments

Please call the office to make an appointment in advance. We will make every effort to accommodate your scheduling needs. We ask that you notify us as soon as possible if you are going to be unable to keep your appointment so we can give that time to another patient.

Fees & Payments

We must request payment arrangements for all services at the time they are rendered unless prior arrangements have been made. For your convenience we accept cash, check, Visa and MasterCard. We will file your insurance claims and we will collect at time of service or bill you for your co-pay, deductible and/or co-insurance per your plan benefits and our contract with your payer. Your medical care is always our priority so we ask that you help minimize the time and effort we have to spend on billing by paying promptly or speaking to our staff as soon as possible to make payment arrangements. We will gladly assist you in any way possible.
